How to Upload a COVID Vaccine Card into ADP

  1. Download the ADP app on your phone or see below for instructions to use Office Tablets*
    Once you log in, the Vaccine Survey will automatically pop up
     
  2. Answer the Question: Have you received a COVID-19 vaccination?
    • Yes, I am FULLY vaccinated
      1. Yes, I’m Fully Vaccinated – Step 1
        Enter the date or your complete vaccination.
        Note: If you received your booster shot, enter the date you received your booster.
        Also, if you have two vaccination cards (1 for your COVID vaccine and 1 for the booster – write the booster information on your original vaccination card. ADP will not allow multiple uploads at this time)
      2. Yes, I’m Fully Vaccinated – Step 2
        After entering the date, please add the photo of your vaccination.
        You’ll be prompted to take an image of the front of your vaccination card and the back of the card.
      3. Yes, I’m Fully Vaccinated – Step 3
        Once you upload both sides of your vaccination card you’ll see a thank you screen
        Thank you for uploading your COVID vaccination card!
    • Yes, I am PARTIALLY vaccinated
    • No, I have NOT received a vaccination
    • I prefer not to answer at this time

If you selected: Yes, I’m PARTIALLY vaccinated; No, I have NOT received a vaccination; or I prefer not to answer at this time

You will then be prompted to report documentation of a negative COVID test.

At this time, we are not requiring documentation of negative a COVID test, but due the emergency order, we may in the future.

*If you do not have the ADP app downloaded on your phone and you work at Nela, Ivanhoe, HVL, or Del Rio:

Received your COVID Booster Shot? Great! Here are the steps to update your vaccine card in ADP with your booster information

Step 1: Email Jen Amos @ jennifer.amos@gecurrent.com letting her know you received your booster and need to update your card in ADP

Step 2: Jen will send you a new vaccine survey through ADP

Step 3: Follow the steps outlined above by clicking that you are Fully Vaccinated, enter the date of your booster shot, and upload your ORIGINAL COVID vaccine card again, with the booster information noted on the original card.

Note: you can only upload one image into ADP at a time, therefore if you have a separate card for your booster, simply print your booster information on the bottom of your original COVID vaccination card and upload that card into ADP
